Hollie Jacobsen joined Pinnacle Family Medicine in 2014. As our Project Manager she manages care gaps to ensure value based care, closing care gaps and engaging patients in preventative screenings.
Hollie is a native of Western North Carolina where she first began her career in the medical field. She has worked in Health Care for 25 years and has both clinical and clerical experience.
After six years of church planting in the valley, God called Hollie and her family to SE Wisconsin, where she currently resides with her husband and youngest of four children. Hollie enjoys time with friends and family, road trips, sports and FaceTime with her grandchildren.
